Communication and working agreements

On this page, you will find the communication and working agreements of KinderOncoNet. These agreements form the basis for effective collaboration between the (pediatric) physical therapists at the Princess Máxima Center and the affiliated (pediatric) physical therapists close to home.

As an affiliated (pediatric) physical therapist with KinderOncoNet, you may receive referrals for children who are still undergoing treatment at the Princess Máxima Center, as well as for children and young adults who had cancer during childhood.

When a child who is still being treated at the Princess Máxima Center is referred to you, you can expect the following:

  1. The (pediatric) physical therapist at the Máxima Center, in consultation with the patient, you, and the parents, determines whether there is an indication for physical therapy in the practice or for home-based treatment, and which treatment frequency is appropriate.

  2. The (pediatric) physical therapist from the Máxima Center contacts you for a verbal handover by phone and also sends a written handover. You will receive this within one week after the child’s discharge. The letter is also always sent to the general practitioner. Parents can access the letter in the patient portal.

Handover

The handover includes:

  • patient details

  • the medical diagnosis and any additional relevant information

  • the course of the oncological treatment, supplemented with all relevant information from the medical record

  • the current status of motor functioning

  • the reason for the indication for treatment in the community setting

  • treatment recommendations

  • contact details of the Sports and Exercise Center

  • agreements regarding contact and evaluation moments during the course of treatment and at the end of treatment, either by phone or in writing

When children or young adults who are no longer receiving treatment at the Máxima Center are under your (pediatric) physical therapy care, it is of course still possible to contact a (pediatric) physical therapist from the Princess Máxima Center for questions or additional support. Although the (pediatric) physical therapist from the Máxima Center does not have lead responsibility for this treatment, information and advice can always be requested.
